Senior Care – Simplified

Care Planning

Each senior and their family will receive an individually-tailored Care Plan addressing all areas of their family’s needs. This Care Plan will be continually updated as the care needs change for your loved one. Along with caregiver support and referral information for third-party geriatric clinicians and resources, your personal Care Plan will also include information and guidance on the following topics:

Medical / Nutrition
Emotional / Physical
Social / Recreation

In-Home Services

Bring the help directly to you or your loved ones home! A care manager from our team will visit you or your loved one’s residence (personal or assisted/independent) and provide one-to-one assistance with all of the overwhelming tasks needed in the home. This can not only make their residence safer, but also help your loved one to stay in their home longer while maintaining their independence.

"Kate's in-depth knowledge about seniors and her warm personality are priceless. Her focus on the individuals and families needs comes first. Kate was able to help us navigate a transition of changing residences, lifestyle and supports and did it seamlessly. She zeroed in on my own Dads need for social interaction and respite for my mom and came up with a plan that worked for everybody."
